Job Summary

The Hospice Maui Chaplain/Spiritual Care Counselor is responsible for coordinating, planning, and providing spiritual care services to patients, families, and caregivers in keeping with their belief systems, either directly or through coordination of care. In addition, this position acts as a liaison between local clergy and the interdisciplinary team.
Essential Job Functions/Responsibilities
- Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner
- Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs
- Provides direct spiritual care to patients and families/caregivers
- Serves as liaison and support to community clergy and spiritual counselors
- Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision
- Documents direct services in electronic medical record. Familiarity with completing a spiritual needs assessment and developing a plan of care
- Documents ongoing communication with community clergy and spiritual counselors
- Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group. Offers a spiritual opening at the beginning of patient care conferences
- Provides consultation, education, and support to the interdisciplinary group on spiritual care
- Recruits community clergy, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care
- Actively participates in quality assessment performance improvement teams and activities
- Provides for funeral or memorial services for patients as requested
- Works with Hospice Maui staff in planning periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team
- Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers
- To be a spiritual resource to team members and assist them in their growth or in dealing with their grief
- Maintains confidentiality of business and health information in accordance with HIPAA and state regulations and Hospice Maui policies
- Accepts and performs other related duties and responsibilities as required
Position Qualifications (Knowledge, Skills and Abilities)
Education
- Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred.
Experience
- Professional pastoral counseling in healthcare, home care, hospice or palliative care setting required.
An equivalent combination of education and experience, which provides proficiency in the areas of responsibility listed above, may be substituted for the above education and experience requirements.
License/Certification
Other Qualifications
- Hawaii resident preferred.
- Knowledge of and commitment to hospice philosophy of care.
- Demonstrated expertise in dealing with grief/loss issues and crisis intervention.
- Experience working in a culturally diverse setting and comfortable with an interfaith community.
- Excellent verbal and written communication skills.
- Strong interpersonal and collaboration skills to build effective working relationships with patients, families, co-workers and individuals external to the organization.
- Ability to work flexible hours to accommodate evening bereavement support groups and the schedules of grieving family members.
- Personal automobile in good working order and insured in accordance with state/organization requirements.
